Apply For Employment

Applicants must have a minimum amount of experience of at least one year in a hospital setting in their respective field before being considered to join our team.
The application process includes completion of the following forms and/or skills assessments:

 
- Employment application
- Statement of Confidentiality
- HBV Vaccination Consent
- Reference Check
- Credit Check Release
- Driving Record Release
- Emergency room nursing (for staffing applicants only)
- Certified Nursing Assistant skills assessment
- Critical Care Nursing
- Medical Surgical Telemetry and LPN (for staffing applicants only)

We also conduct an in person interview and at that time, applicants must present the following items to be copied and included as part of the application process:

 
- Current CPR certificate
- Result of Tuberculin Test
- Driver's license
- Two signed reference checks
- Social Security Card
- Physician Health Statement
- Hepatitis B Vaccine
- Chest x-ray result
- Proof of immunity to Rubeola/Rubella
- Professional License or Certificate
- Car Insurance (for Home Health applicants)
- Documentation of training received related to specific profession in prior 12 months
